Ambulatory Care Network

The Ambulatory Care Network (A.C.N.) makes provides access to high-quality, evidence-based, friendly outpatient care within our local community, regardless of ability to pay. As part of the NewYork-Presbyterian Division of Community and Population Health, we are committed to improving the physical, behavioral, and social health and well-being of the communities we serve with the goal to achieve healthcare equity for all. The goal of the Division of Community and Population Health is to reduce and ultimately erase health disparities by linking our neighbors with the world’s best healthcare services. We work to accomplish this through initiatives that:

  • Enhance the quality of life for our community

The A.C.N. West/Allen Hospital Internal Medicine group delivers adult primary care at 3 practices throughout upper Manhattan. Our practices are National Committee for Quality Assurance (NCQA) Level 3 Patient Centered Medical Homes. Our faculty lead efforts in medical trainee education, quality improvement, behavioral health integration in primary care, integrative medicine, addiction medicine, immigrant health, and community and population health work.
